To sign up, you must come in person to the LifeSource facility at 600 S. School Ave. Suite 2 Fayetteville, AR 72701. The parent or guardian must bring a photo ID, and some form of identification for each child. For example, a birth certificate, a social security card, a consulate card passport, AR Kids card, medicaid card etc.
